1月に受けたセミナーより。特異的な所見はないが、下記のような点、とくに下線を引いた項目を重要視して判断する。
①Diffuse inflammation
②Cryptitis…好中球が陰窩に浸潤した像
③Crypt abscess …好中球が陰窩に集簇した像
④Basal plasmacytosis…基底膜直上の形質細胞増多症
⑤Crypt-associated granuloma
⑥Capillary proliferation and dilatation with hyperemia
⑦Goblet cell depletion…杯細胞減少
⑧Crypt distortion…陰窩のねじれ。IBD以外ではあまり見られない。
⑨Branching glands
⑩Paneth cell metaplasia…左側結腸には通常パネート細胞はみられないので、左側結腸にパネート細胞化生が見られたらIBDを強く疑う。
⑪Gap between the base of the crypts and the muscularis mucosae…陰窩と筋板の間のギャップ
⑫Dysplasia(adenocarcinoma)
